Connecticut: Foxwoods patron voluntary exclusion (gaming floor only)

Tribal gaming commission Run by Mashantucket Pequot Tribal Nation (MPTN) Gaming Commission.

The Mashantucket Pequot Tribal Nation (MPTN) Gaming Commission runs this exclusion for Foxwoods Resort Casino's land-based gaming facility and any gaming the Commission is authorized to regulate. Terms are Five-Year or Permanent: the Five-Year term is automatically rescinded once, at the five-year mark, and once you have used that one-time allowance any later request is restricted to Permanent. This is a tribal program covering Foxwoods property. It is separate from Connecticut's statewide DCP list for online gaming, sports wagering, online lottery and fantasy contests, and separate again from Mohegan Sun's own program — enrolling here does not put you on either.

What it blocks

Covers Foxwoods Resort Casino's land-based gaming facility and any gaming the MPTN Gaming Commission is authorized to regulate. The form does not address whether it also excludes from Foxwoods-branded online/mobile products; those are covered separately by the statewide DCP list.

Self-exclusion programs do not all block the same things. Here is what the official rules say about each one.

How to enroll, how long it lasts, and the sources

Terms you can choose

Five-Year (automatically rescinded once, at the five-year mark — a one-time allowance) or Permanent (irreversible; any later request after using the five-year option is restricted to Permanent).

Sign up for the longest exclusion time you can. Future you will thank you.

When it ends

A fixed term ends on its own at the end of the term; a lifetime term has no published way to lift it.

A fixed term. A Five-Year exclusion is automatically rescinded once, at the five-year mark, with nothing required from you — a one-time allowance. Any later request after you have used it is restricted to Permanent. No process is published for ending a Five-Year term before the five years elapse.

A lifetime term. Permanent is one of the two terms the form offers and is irreversible; no removal process is published for it.

How to enroll

Mail the notarized form to the MPTN Gaming Commission.

Complete the form, sign it in front of a notary, and mail it with a passport-size photo or a clear copy of your driver's license to the MPTN Gaming Commission Chair. You can instead complete it in person at the Gaming Commission Office at Great Cedar Casino, where an MPTNGC representative can witness your signature and no notary is needed. Either route is how you get onto the Foxwoods exclusion list, which covers Foxwoods and the gaming the Commission regulates. It does not put you on Connecticut's statewide DCP list for online gaming, sports wagering, online lottery and fantasy contests, and it does not reach Mohegan Sun. Nothing published says what the Commission sends back or how long it takes, so follow up with the Gaming Commission Office and ask whether your exclusion is recorded. We don't fill in or submit this form.

  1. Download the Patron Voluntary Exclusion form (Form I-100A, Rev. 1-2-14). The state DCP's own self-exclusion page links to it directly. We don't fill in or submit this form.
  2. Fill in your name and address, including province/state and country, and choose Five-Year or Permanent.
  3. For the mail route, sign in front of a notary and include a passport-size photo or a clear copy of your driver's license. If you complete the form in person at the Gaming Commission Office, an MPTNGC representative can witness the signature instead and notarization is not required. Whether a remote online notarization is accepted is not addressed.
  4. Mail it to MPTN Gaming Commission Chair, 350 Trolley Line Boulevard, P.O. Box 3250, Mashantucket, CT 06338-3250, or bring it to the Gaming Commission Office at Great Cedar Casino, at Foxwoods.
  5. No processing time and no confirmation method are published, so nothing tells you the form arrived or that the exclusion is recorded.
  6. A Five-Year exclusion is rescinded automatically once, at the five-year mark — a one-time allowance. After you have used it, any later request is restricted to Permanent, and Permanent is irreversible. No process is published for ending a Five-Year term before the five years elapse.
  7. This covers Foxwoods and the gaming the MPTN Gaming Commission regulates. The form does not address Foxwoods-branded online or mobile products; those sit under the state's separate DCP list, which is its own enrollment.

Before you count on it. This is a Mashantucket Pequot Tribal Nation program covering Foxwoods, and it stands apart from the state. Enrolling here does not add you to Connecticut's statewide DCP list for online casino gaming, sports wagering, online lottery and fantasy contests, and joining that state list does not exclude you from the Foxwoods gaming floor. It does not cover Mohegan Sun, which runs its own separate program. The form does not say whether it reaches Foxwoods-branded online or mobile products. A Five-Year term is rescinded automatically once at the five-year mark; once you have used that allowance, any later request is restricted to Permanent, which is irreversible, and no early-removal process is published for an active Five-Year term. Residency eligibility, processing time and confirmation are all unaddressed — the form collects province/state and country fields, which implies non-U.S. applicants are contemplated, but no source states an eligibility policy. Mailing the form is a submission, not proof of exclusion, so keep a copy of the form and write down the date.
