Every casino, app and bank has its own form, rules and phone number. We find the ones for where you gamble and put them in order. The letters we write are filled in from your details. Most official forms open blank, the way each agency publishes them. You fill those in, and you send everything yourself. Where we have no confirmed address, the plan says so.
