New York: statewide Voluntary Self-Exclusion (VSE)

State regulator Run by New York State Gaming Commission.

One statewide list covering the four commercial casinos, video lottery gaming facilities, retail and online sports wagering, NY Lottery draw/instant games and QuickDraw, named OTB regions and racetracks, multi-jurisdictional advance deposit wagering, and (through the regulation's 'gaming operator' definition) interactive fantasy sports registrants. Charitable gaming coverage is narrower than the program page's plain-language claim: it reaches only online/mobile raffle ticket sales, not confirmed for bingo or bell-jar/pull-tabs. Online casino play is not covered because it isn't legal in New York outside in-person play at the four upstate facilities. Tribal casinos are outside this list entirely — Seneca and Oneida run their own separate programs.

Reviewed 2026-09-24 against official sources. Every fact below links to the page it came from. How we check facts.

What it blocks

Covers commercial land-based casinos (all 4 named on the form), video lottery gaming facilities (named), retail and online/mobile sports wagering, NY Lottery draw and instant games and QuickDraw, named OTB regions and racetracks plus multi-jurisdictional advance deposit wagering, and (via the regulation's 'gaming operator' definition) interactive fantasy sports registrants. Charitable gaming coverage is narrower than the program webpage's plain-language claim: the regulation's 'gaming operator' definition reaches only raffles conducted under General Municipal Law 189(16), and that subsection, read verbatim, is specifically about online/mobile raffle-ticket sales -- no source establishes that bingo or bell-jar operators are bound by Part 5402. The enrollment form's own 'OPTIONAL QUESTION' page lists 'Charitable games (bingo, raffles, pull-tabs/bell jar)' only as an answer choice in a self-reported gambling-habits survey, not as a category a participant selects to be excluded from -- it does not establish coverage. Online casino/iGaming is not covered because it is not currently legal in New York outside in-person play at the four upstate commercial facilities.

Self-exclusion programs do not all block the same things. Here is what the official rules say about each one.

What New York: statewide Voluntary Self-Exclusion (VSE) blocks
What it could stopAnswerWhat the source says

Credit, check cashing and rewardsNot confirmedNo source addresses check-cashing or credit privileges for VSE participants.
Online playConfirmedFor interactive gaming, an operator may not accept new deposits from a self-excluded person, but the person may withdraw any account balance and the operator must acknowledge the withdrawal.
Marketing and mailingsNot confirmedNo source addresses removal from marketing lists for VSE participants specifically.

How to enroll

Open the Request for Voluntary Self Exclusion form.

Complete and sign the official form yourself, then submit it in person at any licensed gaming facility's problem gambling intake area (staff photograph you there) or at Gaming Commission headquarters in Schenectady during business hours, or mail the notarized form with your photo to NYS Gaming Commission, Director of Education & Community Relations, PO Box 7500, Schenectady, NY 12301-7500. We don't fill in or submit this form.

  1. Download the Request for Voluntary Self Exclusion form (Rev. 2/25/2026) from the program page. We don't fill in or submit this form.
  2. Complete the personal-information and physical-description sections, initial your chosen term (one, three or five years, or lifetime — it cannot be shortened once selected) and the acknowledgment section, and sign. Attach a recent photo (2x2 to 4x6 inches, taken within the last six months, full face from the neck up, no tinted glasses or hats); a facility takes this on site if you complete the form there.
  3. If you are mailing the form, sign it in front of a notary — required for mail submissions only, not for in person. The New York Council on Problem Gambling's VSE Support Program offers free help completing the form and notarization at 518-903-0217 or VSESupport@nycpg.org.
  4. Submit in person at any licensed gaming facility's problem gambling intake area or at Gaming Commission headquarters in Schenectady during business hours, or mail the notarized form with your photo to NYS Gaming Commission, Director of Education & Community Relations, PO Box 7500, Schenectady, NY 12301-7500.
  5. A one, three or five year term comes off automatically at the end of the term unless you request an extension in writing at least 30 days before it ends. No removal process is published for the lifetime term.

Before you count on it. Charitable gaming coverage is narrower than the program page suggests: this reaches only online/mobile raffle ticket sales, not bingo or bell-jar/pull-tabs. Online casino play isn't covered — it isn't legal in New York outside in-person play at the four upstate casinos. Tribal casinos (Seneca, Oneida, and the Saint Regis Mohawk Akwesasne Mohawk Casino) are not on this list — Seneca and Oneida run their own separate programs, and no self-exclusion program was found for Akwesasne. Whether residents or visitors qualify isn't stated. No processing time or confirmation method is published, so completing or mailing the form doesn't by itself confirm you're excluded.
