How to enroll
Open the Maverick self-limit form.
Hand the completed form to the cage at any Maverick casino, or email it to nnv.selflimit@wsggaming.com. The form says an emailed copy must be notarized. Maverick gave us that address in writing on September 29; the form still shows an older address that no longer accepts mail.
- Complete the October 2025 form: name and any nicknames, date of birth, contact details, driver's license number and state, and your Play Maverick Club number. Initial each of the seven acknowledgments, then sign and date. We don't fill in or submit this form.
- Hand the completed form to the cage at any of the four Maverick casinos. This is the confirmed route.
- Or have the signed form notarized and email it to nnv.selflimit@wsggaming.com. Maverick's compliance lead gave us this address on September 29, 2026. The form still lists nnv.selflimit@maverickgaming.com, which no longer accepts mail, so don't use that one.
- The request takes effect within 14 days of receipt. The form does not say how Maverick confirms enrollment, so keep a copy of what you sent and write down the date.

The October 2025 form names nnv.selflimit@maverickgaming.com for notarized requests, but that address rejected our messages on 2026-09-10 and 2026-09-29 ('550 5.4.1 Recipient address rejected: Access denied'). On 2026-09-29 Maverick's compliance lead answered in writing: 'Please try, nnv.selflimit@wsggaming.com we are currently updating our forms to include the updated email address.'
