Terms you can choose
Enrollment is a minimum of one year and continues until MGM approves a withdrawal request.
When it ends
This continues until a removal is granted.
The form states enrollment continues until MGM approves a withdrawal request, after a minimum of one year. Nothing ends on its own, and it is the approval rather than the request that ends it.
How to enroll
Open the MGM Self-Limit form.
Hand the completed form to the casino cage at any MGM Resorts property, where no notary is needed, or have it notarized — in person or online — and send it by email or mail.
- Complete the July 2026 form: name, date of birth, MGM Rewards number, contact details, and driver's license number, state and expiration. Sign and date it. We don't fill in or submit this form.
- Choose how you will submit. Handing it to the casino cage at any MGM Resorts property needs no notary. Email or mail requires a notarized form. MGM told us in writing that the notarization can be done online, by any U.S.-commissioned notary, and that you do not attach a copy of your ID.
- Email the notarized form to self-limit@mgmresorts.com, or mail it to MGM Resorts Responsible Gaming Corporate Compliance Department, Attn: Self-Limit, 71 E. Harmon Ave., Las Vegas, Nevada 89109.
- Allow up to 14 days for processing. MGM emails you a confirmation once it has received and processed the form. Keep a copy of what you sent. If nothing arrives, ask for your status at self-limit@mgmresorts.com.
Official form (UPDATED 07/27/2026) Program page
Before you count on it. This is a self-limit, not an entry ban: cash play and hotel stays remain allowed. Residency eligibility is not stated. Preparing or sending the form does not confirm enrollment. MGM's confirmation email is what tells you it is done.

Requirements and sources
Sources read 2026-09-25. These are the program's requirements, not a record of anyone's enrollment.
- Terms available
- Enrollment is a minimum of one year and continues until MGM approves a withdrawal request.
- Can a resident enroll
- Not verified: The form offers email, mail and casino-cage sign-up and does not state a residency requirement.
- Can a visitor enroll
- Not verified: You can hand the form in at any MGM casino cage, and the form does not state a residency rule for visitors.
- Identification
- The form requires identity/contact fields, including driver's license number, state and expiration. MGM Corporate Compliance answered in writing on 2026-09-24 that "A copy of an ID is not required when enrollment forms are notarized." What proof of identity the notary itself asks for is set by the notary's own jurisdiction and is not MGM's requirement.
- Notarization
- Notarization is required if the form is submitted via mail or email; delivery in person at the Casino Cage requires no notarization. MGM Corporate Compliance confirmed in writing on 2026-09-24 that the notarization may be performed "either in-person or online" and that "All U.S.-commissioned notaries are accepted to verify identity."
- Witnesses
- Not verified: No witness requirement is stated one way or the other.
- Notarizing online
- MGM Corporate Compliance confirmed in writing on 2026-09-24 that both email and mail submissions accept an online notarization: "Self-Limit Enrollment Forms will be accepted by e-mail as long the document is notarized, either in-person or online," and the same rule applies by mail. Any U.S.-commissioned notary is accepted ("All U.S.-commissioned notaries are accepted to verify identity"), so you do not need a Nevada notary. MGM's reply does not specify a particular online-notary vendor or certificate wording.
- How long processing takes
