Terms you can choose
Minimum two years. Reinstatement is not automatic and requires written PBGC approval before entering any property; a person may only request reinstatement if they have received no more than one prior voluntary exclusion and at least two years have passed since the exclusion's issue date. A second or subsequent voluntary exclusion results in an involuntary lifetime exclusion.
When it ends
This continues until a removal is granted.
The exclusion stays in effect until the person receives written PBGC permission otherwise. Reinstatement requires a written request the Commission must approve, available only after the two-year minimum and only if the person has no more than one prior exclusion.
How to enroll
Email the form and ID selfie to GC.Investigators@PokagonBand-nsn.gov.
Attach the completed exclusion request form and the required 'ID selfie' -- a clear photo of yourself, shoulder to top of head, holding a valid photo ID. Sending the email doesn't exclude you by itself: the exclusion takes effect once the Commission approves it, and stays in effect until you get written permission otherwise.
- Download the PBGC Casino Voluntary Exclusion Request (Rev04.1) from the tribe's website. We don't fill in or submit this form for you.
- Fill in your name, contact and address details, date of birth, player account ID, the last 4 digits of your SSN, and your license or ID number. Sign and date it.
- Take an 'ID selfie' -- a clear photo of yourself from shoulder to top of head, holding a valid photo ID legible in the shot -- and attach it to the form.
- Email the form and the ID selfie to GC.Investigators@PokagonBand-nsn.gov. The exclusion takes effect once the Commission approves it and stays in place until you get written permission otherwise. The minimum term is two years; reinstatement after that needs its own written request, which the Commission has to approve.
Before you count on it. This form only reaches Four Winds South Bend's gaming floor, hotel, on-site venues, and parking, plus the tribe's three Michigan Four Winds properties. It doesn't touch the Four Winds online casino, which the form leaves out. Four Winds runs a separate online exclusion for it, through your online account or a form on its own website, and its own page ties the online casino to Michigan. This program is run separately from Indiana's state-run VEP (a different card here). The Band's page says it recognizes every Indiana exclusion, VEP included, so your VEP enrollment already covers its casinos. Nothing published says a Pokagon exclusion puts you on the IGC list. A second exclusion becomes an involuntary lifetime bar. You can also ask in person at the Security department of any Four Winds casino for the Gaming Commission. The Band's page says a remote request may go by email, fax or US mail, but only the email address is published for exclusion requests. Filling out or emailing the form doesn't confirm the exclusion; it takes effect only once the Commission approves it.

Routes that enroll you
- By email — GC.Investigators@PokagonBand-nsn.gov
Submit the completed exclusion request form together with the required 'ID selfie'.
Routes that get you a form or an answer, not an exclusion
- By email — GC.Investigators@PokagonBand-nsn.gov
Same address also accepts reinstatement (removal) requests; the reinstatement form gives fax and mail as alternatives. - By mail — Pokagon Band Gaming Commission, ATTN: Reinstatements, 10528 Maudlin Rd, New Buffalo, MI 49117
Published on the reinstatement (removal) form as an alternative to email or fax; this Michigan address is the Commission's own office, not evidence of a wrong-recipient error even though the exclusion also covers the Indiana property. No mailing address is published on the exclusion (enrollment) form itself -- email is the enrollment channel it names.
