Terms you can choose
Applicant checks one of: one (1) year, five (5) years, or ten (10) years. 'This Self-Exclusion Form request is irrevocable during the length of time indicated above. Once this paperwork is completed and submitted, the Self-Exclusion Form will be in effect for the entire time requested. The length or effectiveness of the Self-Exclusion Form is not negotiable and absolutely no exceptions will be made.' No post-term removal procedure is described.
When it ends
Not verified.
The form fixes a term of one, five or ten years and calls it irrevocable, and it says nothing about what happens when that term runs out. No renewal or removal process is published anywhere. A date you write down proves nothing about when this ends — ask the Choctaw Nation Gaming Commission.
How to enroll
Mail the notarized form to the Choctaw Gaming Commission.
The form says to mail or deliver it, with your photograph attached, to this P.O. Box. Your signature has to be notarized and a copy of your driver's license or government-issued photo ID has to go with it. Nothing is published about a receipt or acknowledgment, so keep your own copy and proof of mailing. Expect up to 60 days before your name cycles out of Choctaw's mailings.
- Download the self-exclusion form from the Choctaw Casinos compliance page. We don't fill in or submit this form for you.
- Complete your full name, date of birth, any aliases or nicknames, mailing address, telephone number and physical description, including scars and tattoos.
- Attach a copy of your driver's license or government-issued photo ID, and a photograph.
- Check your term: one, five or ten years. The form states this is irrevocable for the period you pick and that absolutely no exceptions will be made.
- Sign in front of a notary — the form ends with a full notary block.
- Mail or deliver the completed form with the photo attached to Choctaw Nation of Oklahoma Gaming Commission, P.O. Box 5229, Durant, Oklahoma 74702-5229.
- Expect up to 60 days before your name cycles out of any Choctaw Casinos mailings you currently receive. You may not use any offer you received after the date you signed.
Before you count on it. This covers Choctaw Casinos & Resorts properties and nothing else in Oklahoma. The term you pick is irrevocable and the form says no exceptions will be made, and nothing published describes what happens when it ends — no renewal or removal process appears anywhere. If you are identified trying to enter or use the facility you will be escorted out, and continued attempts may put you on the involuntary casino trespass list. Money you win while excluded is forfeited at $100 or more in promotional items, tickets or chips. The Nation and its Gaming Commission disclaim liability for failing to enforce the exclusion and do not waive sovereign immunity, so being on this list is not a guarantee you will be stopped at the door. No receipt or confirmation is published. Whether a remotely notarized signature is accepted is not established. Whether someone living outside Oklahoma may enroll is not established.
