Oklahoma: one form OAPGG forwards to the casinos on its list

Shared program Run by Oklahoma Association on Problem Gambling & Gaming (OAPGG).

Oklahoma has no state self-exclusion list. What exists is a shared intake: the Oklahoma Association on Problem Gambling & Gaming, a nonprofit, takes one notarized application and forwards it to the 17 entries named on its current form — tribal gaming commissions plus Remington Park. OAPGG has no legal authority to enroll or remove anyone. Whether each recipient accepts and enforces your request is that tribe's own decision, under its own compliance rules. You pick 1, 3, 5 or 10 years, and OAPGG cannot change it once you are on the list.

Reviewed 2026-09-24 against official sources. Every fact below links to the page it came from. How we check facts.

What it blocks

A single application that OAPGG forwards to a numbered list of 17 entries — not every Oklahoma gaming tribe, and the count of distinct tribal authorities among those 17 is itself unresolved. 'OAPGG recognizes the need for some individuals to request self-exclusion from multiple casinos... Please note, not all tribes participate in the self-exclusion program but may choose to in the future.' OAPGG itself is a nonprofit with no legal authority to enroll or remove anyone; it is an intake and forwarding service, and enforcement depends on each recipient tribe's or Remington Park's own compliance. A 2026-09-24 re-extraction of the current form's contact list found: Absentee Shawnee, Cherokee Nation, Chickasaw Nation, Choctaw Nation, Citizen Potawatomi Nation, Eastern Shawnee Tribe, Kaw Nation, Kiowa Tribe, Miami Nation, Otoe-Missouria, Ponca Tribe, Sac & Fox Nation, Seminole Nation, Shawnee Tribe, and Remington Park — plus two entries under the same phone number (918-995-8400), 'Creek Nation Gaming Commission' and 'Muscogee Creek Office of Public Gaming', which appear to be the same tribal government listed twice under two different office names rather than two separate participants; this is recorded as an open question, not resolved. A completed Choctaw Nation form separately states it treats an OAPGG enrollment as effective at all Choctaw Casinos, so the two programs are not fully independent for that one tribe.

Self-exclusion programs do not all block the same things. Here is what the official rules say about each one.

Terms you can choose

Applicant checks one of: 1 year, 3 years, 5 years, or 10 years. 'Please note that once you are added to the self-exclusion list, the timeframe cannot be changed.' The form separately states: 'This Self-Exclusion request is IRREVOCABLE by OAPGG for the time specified.' Removal before term expiry, or after it, is not something OAPGG can do on its own: the applicant must contact each participating tribe's Gaming Commission individually, and some Commissions will not remove, modify, or alter the application once accepted.

When it ends

This continues until a removal is granted.

OAPGG cannot take you off. To come off — before your term ends or after it — you have to contact each participating tribe's Gaming Commission individually, and each one decides. Some will not remove, modify or alter the application once they have accepted it.

How to enroll

Mail the notarized form to OAPGG in Norman.

The mailing address is printed on the form. Two other routes exist: OAPGG will help you complete it in person at that Norman office if you call 405-801-3329 ahead, or you can submit it online and OAPGG will schedule a short online notary session to confirm who you are. Whichever route you use, OAPGG forwards your application onward — that forwarding is not the same as each casino accepting it. OAPGG says it processes forms within 24 hours of receiving them and mails you a letter when it accepts and activates yours. Nothing published says when each casino adds you or whether any casino confirms.

  1. Download the OAPGG self-exclusion form from oapgg.org, or ask OAPGG to help you complete it. We don't fill in or submit this form for you.
  2. Complete your identifying information and attach a copy of a current, non-expired driver's license plus a separate recent photograph — not the license photo, and no hats or sunglasses.
  3. Check your term: 1, 3, 5 or 10 years. Once you are added to the list the timeframe cannot be changed, and the form calls the request irrevocable by OAPGG for that period.
  4. Get your signature notarized. If you submit online, OAPGG will schedule a short online notary session to confirm your identity.
  5. Send it: mail to Oklahoma Association on Problem Gambling & Gaming, 501 E. Alameda St. Suite E, Norman, OK 73071; or go to that office in person after calling 405-801-3329 to make sure someone is there; or use OAPGG's online submission.
  6. Read the list of recipients printed on the form before you decide this is enough. If a casino you actually go to is not on it, this application never reaches that casino.

Before you count on it. OAPGG forwards your application. It does not enroll you, and forwarding is not the same as a tribe accepting you onto its own barred-persons list — each recipient decides that under its own rules. OAPGG says it mails you a letter when it accepts and activates your form, but nothing published says a tribe will confirm that it added you. Your start date is the day your notary signs. If your form reaches OAPGG more than 90 days after that, it starts the day it arrives. The current form lists 17 entries, and whether those are 17 distinct authorities is unresolved: two of them share the same phone number and appear to be the same tribal government listed twice under different office names. Oklahoma has roughly 30 or more compacting gaming tribes, so some tribes you might visit are not on this list at all, and the form itself says the list is updated periodically. Removal is not something OAPGG can do: you would have to contact each participating tribe's Gaming Commission individually, and some will not remove, modify or alter the application once accepted. The Choctaw Nation treats a completed OAPGG form as effective at all Choctaw Casinos, which is the one reciprocal arrangement recorded here. This does not reach the Oklahoma Lottery, which runs its own separate program. Whether someone living outside Oklahoma may enroll is not established.

  • Online — OAPGG's electronic intake, followed by a scheduled online notary session

Routes that get you a form or an answer, not an exclusion

  • By phone — 405-801-3329 (OAPGG office)
Scheduling/inquiry line for the in-person route; not itself a submission channel.
  • In person — Each participating Tribal Gaming Commission individually (17 entries listed on the form)
This is the removal route, not enrollment: 'If the applicant wishes to have their Self-Exclusion form rescinded for any reason they must contact the Tribal Gaming Commission of each participating tribe individually.'

How long processing takes
OAPGG's written procedure says forms “will be processed within 24 hours of receipt at OAPCG”. That covers OAPGG's intake only. Nothing published says how long each tribe or Remington Park takes to add you. The start date is the date of the notary's signature, and a form received with a notary date more than 90 days old is entered with the date it was received.
How you are told it took effect
OAPGG's written procedure says: “Upon acceptance, OAPCG will mail the applicant a letter of acceptance and notice of the activation of the Self-Exclusion form.” That is OAPGG's acceptance. Whether any participating tribe or Remington Park separately confirms receipt is not published.

The form asks for: identifying information, copy of current (non-expired) driver's license, separate recent photograph (not the license photo; no hats or sunglasses), selected term of 1, 3, 5, or 10 years, notarized signature.
