Maine: Hollywood Casino Bangor and Oxford Casino

State regulator Run by Maine Gambling Control Board / Gambling Control Unit, Department of Public Safety.

Maine's casino and slot-facility exclusion list, run by the Gambling Control Unit. One form covers both of the state's licensed commercial casinos and their slot machine floors. You sign it in person in front of a State official, or a casino security representative if no State official is there, and they take your photo on the spot. Terms are 1, 3 or 5 years, or lifetime; a lifetime enrollee may petition the Board for removal after five years. Sports wagering, tracks and off-track betting sit on a separate Maine list with its own form. If you cannot get to one of the three locations, Maine also lets you request it in a scheduled video meeting with its problem gambling services provider.

What it blocks

Covers Maine's two licensed land-based commercial casinos and their slot machine facilities: Hollywood Casino Bangor and Oxford Casino. Chapter 13 (eff. 10/4/2025) frames a broader 'Universal exclusion list' concept that can also reach advance deposit wagering, fantasy contests and sports wagering operators, and requires an application to let a person choose which activities to exclude from (sec.3(3)(A)(3)); the live MGCB-2100 form (Rev. 08/13/2024) predates that amendment and has no such field, so in practice this form only enrolls a person against casinos and slot facilities. Whether or how a person reaches the broader scope Chapter 13 describes is unresolved.

Self-exclusion programs do not all block the same things. Here is what the official rules say about each one.

What Maine: Hollywood Casino Bangor and Oxford Casino blocks
What it could stopAnswerWhat the source says
Entering the propertyConfirmedCasinos must refuse entry to an excluded person; a person on the list who enters may be arrested for criminal trespass.
Betting and winningsConfirmedA person on the list may not collect winnings or recover losses; any winnings found on or about the person are forfeited to the Gambling Control Board.
Credit, check cashing and rewardsConfirmedSelf-exclusion ends check-cashing privileges, player club membership and complimentary goods/services (Chapter 13, sec.7(4)(E)); the instructions describe the same as ending 'any privileges they have with the casino such as check-cashing services.'
Online playConfirmedChapter 13 requires operators to refuse an excluded person access to a 'digital platform or application' as well as the physical gaming floor (sec.7(4)(B)), so the restriction reaches a casino's own online products, not only its premises.
Marketing and mailingsConfirmedOperators must deactivate any player club card or account and ensure excluded persons receive no emails, solicitations, telemarketing promotions, player club materials or other promotional materials relating to the gaming activity (Chapter 13, sec.7(4)(D) and (F)).

How to enroll, how long it lasts, and the sources

Terms you can choose

1, 3, or 5 years, or lifetime (Chapter 13, sec.3(3)(A)(2)); a term expires on the last day of the month of the original exclusion. A lifetime enrollee may petition the Board for removal after 5 years; a term enrollee serves the full period with no way to shorten it, and the term then ends on its own: 'Except for those persons choosing a lifetime self-exclusion, self-exclusion will automatically terminate upon expiration of the period of self-exclusion' (Chapter 13, sec.8(1)(A)). The Board then deletes the name and notifies operators (sec.8(1)(C)).

Sign up for the longest exclusion time you can. Future you will thank you.

When it ends

A fixed term ends on its own at the end of the term; a lifetime term continues until a removal is granted.

A fixed term. Chapter 13 sec.8(1)(A): 'Except for those persons choosing a lifetime self-exclusion, self-exclusion will automatically terminate upon expiration of the period of self-exclusion.' A 1-, 3- or 5-year term ends on the last day of the month it began in, and the Board then deletes your name and tells the operators (sec.8(1)(C)). The Gambling Control Unit's instructions say the same about the expiry date.

A lifetime term. A lifetime enrollee may, after five years, ask the Board in writing for removal. The request must give a detailed explanation, any history of violations, and verification of a reinstatement session with an approved problem gambling services agency (Chapter 13 sec.8(1)(B)). The Board decides; the instructions say you 'can request' removal and do not promise it, and the form you sign says 'my petition for relief may be denied'.

How to enroll

Sign the form in person at the Gambling Control Unit.

Three locations take this form: the Gambling Control Unit in Augusta, Hollywood Casino at 500 Main Street, Bangor, and Oxford Casino at 777 Casino Way, Oxford. Bring a government-issued photo ID showing your name, current address and date of birth. Sign only in front of the State official or casino security representative — a signature you add at home does not count. They photograph you there, or you may bring a photo taken within the last six months. No mail-in enrollment is published.

  1. Download form MGCB-2100 (Rev. 08/13/2024) from the Gambling Control Unit's Casino Forms and Reports page, or get one on site. We don't fill in or submit this form for you.
  2. Complete it in legible black or blue ink: legal name and any other names you have used, physical and mailing address, primary phone, date of birth, height, weight, hair and eye color, gender, and any distinguishing marks. Your Social Security number is optional; your Player Club number goes on it if you have one.
  3. Initial your term — 1, 3 or 5 years, or lifetime. A term expires on the last day of the month it started in.
  4. Bring a government-issued photo ID showing your name, current address and date of birth. A state ID is fine.
  5. Go to one of three places and sign there: Gambling Control Unit, 45 Commerce Drive Suite 5, Augusta; Hollywood Casino, 500 Main Street, Bangor; or Oxford Casino, 777 Casino Way, Oxford. Sign only in front of the State official, or a casino security representative if no State official is present. They photograph you on site, or you can bring a photo taken within the last six months.
  6. Cannot travel? You can book a video meeting with ADEPT, Maine's problem gambling services provider. Its Problem Gambling Services page, which the Gambling Control Unit links as Self-Exclusion, says it emails you an online form and you finish the process on Zoom. It does not say whether people outside Maine can use it.
  7. Your form and photo go to the Gambling Control Board immediately after the visit and your name is placed on the list. If you chose lifetime, you may petition the Board for removal after five years by writing to the Executive Director, Gambling Control Unit, 45 Commerce Drive Suite 5, 87 State House Station, Augusta, ME 04333.

Before you count on it. This list covers Maine's two commercial casinos and their slot facilities. Sports wagering, commercial tracks and off-track betting are a separate Maine list with a separate form (MGCU-8500), so signing this one does nothing about those. The MGCB-2100 form is signed in person with a witnessed signature, and no mail-in route is published. Maine's rule also lets you ask for a virtual self-exclusion meeting, and ADEPT, the state's problem gambling services provider, offers one by Zoom or FaceTime for casino and sports wagering self-exclusion, so you do not have to travel to Maine. Its page does not say whether it serves someone who lives outside Maine, or how your signature is witnessed in the meeting. Maine adopted a broader rule (Chapter 13, effective 10/4/2025) describing a single universal exclusion list that would let one application reach casinos, slots, advance deposit wagering, fantasy contests and sports wagering together; the live form predates it and has no such field, so whether you can request that combined scope today is unresolved. A 1-, 3- or 5-year term ends on its own: Chapter 13 says self-exclusion 'will automatically terminate upon expiration' unless you chose lifetime. No confirmation notice to the applicant is published.

Not stated: whether someone who lives outside Maine may use it, and how a signature is witnessed.
  • By mail Not verified — no mail-in enrollment route is published
The MGCB-2100 route is an in-person visit with a signature witnessed by a State official or casino security representative, and the other described route is a video meeting; no source describes a mail-in enrollment path. No source refuses one either, so mail enrollment is not verified.

Requirements and sources

Sources read 2026-09-24. These are the program's requirements, not a record of anyone's enrollment.

Terms available
Can a resident enroll
'Any person may request self-exclusion pursuant to these rules' (Chapter 13, Sec.3(1)). No residency requirement is stated.
Can a visitor enroll
The same 'any person' language applies; no residency bar is stated. The MGCB-2100 route is a visit to one of three Maine locations. Chapter 13 sec.3(1)(B) and ADEPT's page also describe a scheduled video meeting that needs no trip; no source says whether someone who lives outside Maine may use it.
Identification
A government-issued photo ID (a state ID is acceptable) showing name, current address and date of birth; a photo is taken on site or a recent one (within 6 months) may be brought.
Notarization
Not verified: Neither the instructions nor Chapter 13 mentions a notary. What both describe instead is a witnessed signature: the form must be signed in person before a State official or, in the State official's absence, a casino security representative. No source says outright that notarization is not required, so whether you need a notary is not verified.
Witnesses
Signed in front of a State official or casino security representative, who witnesses the signature; this stands in place of notarization.
Notarizing online
Not verified: Notarization itself is not established as required, so remote online notarization does not arise on the current record. The signature is witnessed in person by a State official or casino security representative.
How long processing takes
Forms and photos are sent immediately after the visit to the Gambling Control Board and the person's name is placed on the list.
How you are told it took effect
Not verified: No source describes a confirmation notice sent to the applicant after enrollment.

The form asks for: legal name and any other names used, physical address (and mailing address if different), primary telephone number, date of birth, height, weight, hair color, eye color, gender, distinguishing physical marks, term of exclusion, initialed, Social Security number, optional, Player Club number, if any.
