Terms you can choose
Permanent: "any additions to the Exclusion List shall be permanent unless otherwise specified by the Executive Director." A person may petition in writing for removal, "not more frequently than annually". The Commission reviews the petition and "reserves the right to take action on the reinstatement without a hearing" when neither the Agency nor the casino opposes; if either does, it schedules a hearing.
Lifetime is the only term this program offers.
When it ends
This continues until a removal is granted.
How removal works. Additions to the list are permanent unless the Executive Director specifies otherwise. You may petition in writing for removal, no more often than once a year. The Commission can approve it without a hearing when neither its staff nor the casino opposes; otherwise it holds one.
On a lifetime term. Permanent is the only term, so the same rule applies: a written petition at most once a year, and the Commission's approval, with a hearing if its staff or the casino opposes.
How to enroll
Enroll through NVSEP.
The NHBP Gaming Commission's page says you may complete the self-exclusion online through NVSEP, and NVSEP's Michigan page lists FireKeepers as a lifetime exclusion. NVSEP fills in the form with you online, checks your ID and sends it to the Commission, and its portal lists Michigan's tribal casinos among the states it registers directly, with no notary meeting — the notary on page 2 of the packet is for the mailed route. Press Self-Exclude Now on the page and sign in with the code NVSEP emails you. Identity checks start with a live face scan — it asks you to turn your head to each side — before it asks your name, and NVSEP says the pictures you submit are stored on your self-exclusion record. NVSEP says it sends you a confirmation once the submission is processed. To do it on paper instead, use the steps below.
- Enroll through NVSEP. The Gaming Commission's page names it as the online route, and NVSEP's Michigan page lists FireKeepers as a lifetime exclusion. You fill in the form online, upload your ID, sign in front of a free video notary if needed, and NVSEP sends it to the Commission. We don't fill in or submit it. To do it on paper instead, follow the steps below.
- Download the self-exclusion packet from the NHBP Gaming Commission. We don't fill in or submit this form for you.
- Fill in pages 1 and 2: your name, address, date of birth, driver's license number, eye color, height, phone and email.
- Take pages 1 and 2 to a notary and sign them there.
- Mail them with a photocopy of your driver's license or other valid ID to NHBP Gaming Commission, Attn: Investigations Department, 11177 E. Michigan Avenue, Battle Creek, MI 49014.
- Or skip the notary: at FireKeepers Casino Hotel, ask for a Gaming Commission investigator, who prepares the application with you. An application picked up at the Security podium still has to be notarized and mailed.
- Keep a copy. The packet doesn't describe a receipt or a confirmation. Call the Investigations Department at 269-841-1080 with questions.
Official form (Permanent Self-Exclusion order and notice, PDF created 2024-02-26) Program page
Before you count on it. This exclusion is permanent unless the Executive Director sets another term. You can petition in writing for removal at most once a year. The Commission decides; it holds a hearing if the Gaming Commission staff or the casino opposes your return. It covers FireKeepers only and is separate from Michigan's statewide Disassociated Persons List; neither the packet nor the regulations say FireKeepers honors that list. Apart from NVSEP's confirmation of a submission made through it, nothing published says how you'll hear that you're enrolled.
Every route we checked
Routes that enroll you
- By mail — NHBP Gaming Commission, Attn: Investigations Department, 11177 E. Michigan Avenue, Battle Creek, MI 49014
"Mail or fax pages #1 & #2 and a valid copy of Driver's License or other form of identification to: NHBP Gaming Commission, Attn: Investigations Department, 11177 E. Michigan Avenue, Battle Creek, MI 49014". No fax number is printed. - In person — FireKeepers Casino Hotel: ask for a Gaming Commission investigator
The Commission page, option 1: "You may come into the Firekeepers Casino and Hotel, request to meet with a Gaming Commission Investigator who will explain the self-exclusion process and prepare your self-exclusion application." The packet's notary step applies to mailed or faxed pages, not to this route. - Online — https://www.nvsep.org/michigan
The Commission page lists: "You may complete the self-exclusion process online through the National Voluntary Self-Exclusion Program (NVSEP)". NVSEP's Michigan page lists FireKeepers Casino Hotel among the tribal casinos it supports, with exclusion for life. NVSEP says it fills in the form online, verifies your ID, arranges a free online notarization through Proof when one is required, and submits the form electronically. Its own sign-up portal lists Michigan's tribal casinos among the states it registers directly, with no notary meeting. Identity checks there start with a live face scan, and NVSEP says the pictures you submit are stored on your self-exclusion record.
Routes that get you a form or an answer, not an exclusion
- In person — FireKeepers Casino Hotel Security podium (blank application)
The Commission page, option 2: "You may obtain a self-exclusion application from the FireKeepers Security podium and follow the steps on page #2 of the application." Page 2 is the mail-in steps: notarize pages 1 and 2, then mail or fax them. Picking up the application does not enroll you. - By phone — 269-841-1080
Investigations Department, per the 2024 packet. Not a submission route.
