Notary rules for self-exclusion forms

Some forms must be signed in front of a notary. Some need it only when you mail the form, and some accept a witness instead. Read the rule for your program before you book a notary.

Covers all 51 state pages and uses the same records they do. Each state shows the date it was last reviewed. How we check facts.

A source sets a notary rule 17 states

At least one program on each of these pages has a notary rule a source states. Each rule is quoted as our record holds it. Many apply to one way of sending the form and not the others.

  • Delaware reviewed 2026-10-01

    • Delaware Statewide Self-Exclusion Program State regulator
Notary: Source says this does not apply. No notarization requirement appears anywhere in Sec. 203-7.15; the form's required elements call for countersignature by an authorized lottery office employee, not a notary.
  • Maryland reviewed 2026-10-02

    • Maryland Voluntary Exclusion Program (VEP) State regulator
Notary: Source says this does not apply. Not required for standard in-person enrollment, which is staff-witnessed with no notary block. Notarization applies only to the discretionary mail-in exception, which must be pre-approved by the Responsible Gaming Program Director. The agency's own overview page (md-vep-overview) documents only in-person enrollment and never mentions a mail option; the mail/notary exception is documented on the Maryland Alliance for Responsible Gambling's partner page instead.
  • Massachusetts reviewed 2026-09-24

    • Massachusetts Voluntary Self-Exclusion (VSE) State regulator
Notary: Source says this does not apply. The form has no notary block. Instead, a 'Designated Agent' (an MGC or casino staff enrolling agent) countersigns; enrollment is completed by a designated agent in person or by scheduled video appointment, not notarized.
  • Rhode Island reviewed 2026-09-24

  • Virginia reviewed 2026-09-24

    • Virginia: lottery accounts, casinos, sports betting, charitable gaming, fantasy contests and horse racing State regulator
Notary: Source says this does not apply. 11VAC5-60-20 comprehensively enumerates the required contents of a self-exclusion request (name, DOB, address, phone, SSN, and 'valid identification credentials containing the individual's signature and a photograph') and includes no notarization requirement. The word 'notary' appears nowhere in any of the six sections making up Chapter 60 (11VAC5-60-10 Definitions, -20 Request, -30 List, -40 Duties, -50 Removal, -60 Forfeiture of winnings), each retrieved and checked 2026-10-01.
